*Reminder: Parents must be approved CPS volunteers. If you are not already approved, please fill out this form and email it to Maribel Quiros in the office along with a copy of your ID and vaccination card.

Details for attending students:
- Eat dinner before you arrive - only take home sweets goody bags will be provided
- Bring a water bottle, old t-shirt (for dog chew toy craft) and a non-perishable food item or $1 donation (buys four meals) for the Common Pantry
- Enjoy dancing, games, photo booth and activities
- Follow all school day covid protocols
